Totally understandable question, as there is a lot on social media and the Internet that GLP-1 agents could be harmful. There are always risks associated with the use of medication. However, these are very limited with GLP-1 medications and having oversight has more health risks than using them.
Here's how we ensure safety and why, with the right patient, the benefits often outweigh the risks:
Contraindications screening. We exclude pregnancy/childhood wish, previous pancreatitis, and (familial) MEN2/MTC, among others, and review your medication list carefully.
Quiet titration + monitoring. We build up step by step and track weight, blood pressure, symptoms, and lab values as needed.
Side effects usually mild and temporary. Consider nausea, feeling full, belching, constipation/diarrhea or abdominal pain in the first few weeks-usually go away with slower accrual or temporary withdrawal.
Clear "sick-day rules" and 24/7 accessibility. In case of persistent vomiting, severe abdominal pain, or other alarm symptoms, switch with us immediately.
In addition, you should realize that:
Obesity itself increases the risk of cardiovascular disease, type-2 diabetes, high blood pressure, stroke, sleep apnea, liver disease/NAFLD, joint disease and various cancers, among others. This is well documented by CDC/WHO and the NIDDK.
Demonstrated risk reduction with GLP-1 in the right target population.

GLP-1 medication starts working as early as one day after injection. You will immediately notice that your sense of hunger decreases and you feel full faster.
You often notice weight loss within 1-2 weeks.
The greatest decrease usually occurs in months 3-6.
On average, you'll see double-digit weight loss over several months, but it remains individual.

Losing weight with GLP-1 injections is safe if under medical supervision. Of course, as with any medication, there are risks with use. With GLP-1 medication, apart from gastrointestinal symptoms, the following (serious) side effects have been described.
- Gallstones/gallbladder inflammation - sometimes (±1-2%)
- Pancreatitis (pancreatitis) - rare (<0.3%)
- Exacerbation of diabetic retinopathy - rare (0.01-0.1%) and especially in diabetes with rapid HbA1c decline.
- Allergic reaction - Rare - very rare (<0.1%)
- Thyroid warning (C cell tumors) - seen theoretically In rats; not demonstrated in humans.

80/20 principle. 80% nutritious (vegetables, fruits, whole grains, legumes, fish/chicken/egg/dairy/plant protein, nuts), 20% is flexible.
Every meal protein + fiber. Think cottage cheese/yogurt, eggs, legumes, tofu, chicken/fish + vegetables/grains. This will prevent muscle loss and keep you full longer.
Smaller, calmer meals. GLP-1 slows gastric emptying; large fatty meals can cause nausea-rather small and quiet meals.
Drink well. Guideline ~1.5-2 liters of water/tea per day.
